My recommended: try setup smtp in your website to know is work or not. Or maybe you can try with other hosting provider to test? Because I'm confirm the contact form work good in my hosting provider. So maybe you can use one month trial host from site5: http://goo.gl/0rtIu5, you can use this coupon: FREETRIAL. So you can see it's work in other hosting provider and not work in your current hosting provider.
If you don't want use site5, you can try: Sitegound or InMotion, both of thems is the best hosting company.
